funny you say that it was tested twice before it was installed
I would have to guess a bad voltage regulator or diodes. I had a 100% new autolite unit pulse my lights at off idle engine speeds. swapped the old alternator back in and it didn't do it.
it also bench tested find, however it was charging alittle high.
I've had better luck with the Ford reman units. or try a low milage junk yard unit, had good luck with one of those before.

I'm having the same issue..... I have an 02.. Old alt. died out by casually fading away.
I replaced with a new Duralast and now I have 1.) High-pitched electric-winding-whistle.. 2.) Severe flickering/pulsing all through-out.
There is no 1-pin connector or middle wire on the 3-pin connector... I've read the big 3 fix does not work on these, if you switch to an f600 or something similar to the 99-00 3-pin clip you can then accomplish the big 3?! Reassurance is nice... haha
But what/where is the voltage regulator at? Any more info. on this would be appreciated.... Thanks!
